This form only needs to be completed by parents / guardians living outside the school catchment area when applying for a September intake Reception place.  Please refer to the school admissions criteria for details.  Please not that this is not an application form.  It will be used in addition to the LA application form and wil allow the school governors to place applications in order, according to the school's admission criteria.
